  • Hello and advice on deed poll form loc020

    Hello, I'm new the forum.

    I need a bit of advice on completing the loc020 deed poll form
    "CHANGE OF NAME DEED intended for enrolling in the Central Office of the Senior Courts of England and Wales"

    I'm aware that enrolment is not a requirement in the UK, but we're getting married abroad and the authorities there insist on an enrolled deed poll.

    On the second page, there is a part that says "I have subscribed my adopted and substituted", with a tick box for forename(s) and surname(s). It's just my surname that's changed, so am I right in thinking I just tick the surname box? (See attached example.)

        Thanks again for your advice. I've done the LOC020: Change of name deed. Next step is a statutory declaration, LOC021: Deed poll statutory declaration for an adult with a solicitor. Am I right in thinking I only need one adult who has known me 10 years? (Unlike with the change of name deed, which required two witnesses.)

            Thank-you. I went to a solicitors and we did the statutory declaration, and the relevant exhibit copies. All straightforward for the princely sum of £9.00. I'll report back on whether I've actually done it correctly when I hear back! :-)

              Hi all, just to say I received my enrolled deed poll. Quite a bit quicker than I expected, and thanks again for the help here.

              I just want to stress this point, I only needed a solicitor for the statutory declaration. A mistake I made at the start was asking solicitors for an enrolled deed poll. All those I asked (and that replied) said they didn't do it. One even suggested that I could not have a deed poll since my name had been previously changed. Aside from the time this took, I suspect if I had found a solicitor willing to complete the paperwork for me, it would have cost me quite a bit more.

                  Hi sorry to piggy back on someones else's post, however I wonder if anyone has advice on the proof section of the form LOC021, I am trying to figure out why is meant by:



                  'The certificate of
                  (NAME THE DOCUMENTATION SHOWN AS PROOF OF CITIZENSHIP)'

                  and

                  'now produced and shown to me and marked "B" is the Certificate of
                  (NAME THE DOCUMENT OF CITIZENSHIP)

                  the documentation is somewhat confusing, I think I am meant to put in Birth Certificate for both box's but not 100% sure.

                  Also, when I got married I changed my surname to be wife-husband, thus it is different from my surname on my bith cirtificate should I include my decree nisi?
