Application form or CCA????

Id be grateful if anyone on here could advise where I stand with this from Lloyds TSB, it appears to be a combined CCA/Application form and I have received it (late!) in reply to a CCA request I have made.

Also does it matter that this application was filled out in a branch at all?

Accompanying this is 6 pages of non specific terms and conditions print out (date effective from Oct. 2002) and a photocopy of small print which appears to be the rear of the application form (see below)

Is this CCA enforceable?
