Have you received anything else with this

ie. any T&C's

What does it say at the top

ie. application form, or credit agreement

It is very hard to decipher on here

is it the same when actuall looking at the document

What are you hoping to acheive here

ie. making a full and final offer for the account

Can you give more details of what you want to acheive and also more details of the account ie. what has happened which has led you to this

is the account been defaulted, assigned or is it still with the original creditor

is there any outstanding balance

Also have you sent a SAR yet

we would need more info to advise properly
