M + s personal reserve ppi rejected ticked box - not my tick??

Hello
I wonder if anybody can help me. I have received a rejection letter from M+S Personal Reserve stating I ticked the box on an instore application and the sale was not advised as in store. They have enclosed my application form which is barely legible but there is a tick in the PPI box. I have also ticked the direct debit instruction, which is my tick. The tick in the PPI box is definitely not my tick and is clearly different to the direct debit box tick. Is there anything I can do?
