  • Sick note

I was issued with my first sick note on the 28th December which was for 6 weeks, my consultant then issued a second note yesterday for a further 2 weeks (I still have a week to go from my first note)
Would I loose this week and my sick be takenfromtheissuedateofthesecond note? Or should the second note start after this week?

That will depend on the wording of the second note.

The consultant signed and dated the note but did not put any dates, she just wrote 2 weeks

If you are asking the question to find out what dates your company would take from the second certificate, then without knowing how it has been completed it is difficult to advise.

However if they the Fitness to Work Certificate has been completed with the box "I assessed your case" detailing the date you saw your consultant and then in the box "This will be the case for" and they put two weeks and left the boxes "or form" and "to" blank then the two weeks will be taken from the date you were assessed. This second Fitness to Work Certificate will supersede the first one.

        Hope that makes sense.
