Older thread, but just wanted to say that my experience with both of my elderly parents is that GPs and hospital consultants are very reluctant to 1. refer to elderly care psychiatry ( who can diagnose dementia) 2. Stop people from driving, unless the person is suffering from physical or mental health problems that make it obvious that they can't drive , in which case there isn't really a " "decision" to be made! Regarding fitness to drive assessments, some driving instructors do them, but they only involve assessing the driving.
There are centres where trained assessors including an Occupational therapist and driving instructor carry out a full scale assessment, including assessing physical and cognitive ability, as well as driving.
I insisted my father had one when he hadn't driven for 8 months following a prolonged rehab period after hip fracture and shoulder damage.

Getting anyone to tell him he had to stop driving 2 years later after 2 accidents, the last one of which he couldn't offer any explanation for ( no other car involved. Arrived home with 3 flat tyres and had bent 2 of the alloys beyond repair) wasn't so easy. Eventually 3 months later he had a small stroke and it was discovered that his license had expired 2 months before, so GP said they couldn't support him in his renewal.
