Name Change by Deed Poll

Hi,

I'm a little confused filling in a deed poll form.

Do I need 2 people to witness my name change plus someone who's known me for 10 years?

For the LOC021 form it says about witness solicitor so does this person who's known me for 10 years need to get this filled in with a solicitor witness?

He doesn't need to have it filled in by a solicitor, but just signed by the solicitor before whom he swears the information given is correct.
Cost used to be £5, but haven't sworn an affidavit for some years!
