Form FL403 application to extend non-molestation order

Hi, am filling out my own form but have a couple of questions:*
2. It asks for details of the order: What details? The fact that it's a non-mol? The exact conditions?
And do I deliver the order to the family court in person? Together with the original order? There is no guidance whatsoever to go with the form.
