What form do I use and what is the CPR rule to make an application to amend my witnesses statement in civil court Petition for relief, companies act 2006, S994 case? I am a litigant in person, keep getting told its a AII form but that's abut an insolvency matter and nothing to do with this case

I don't know much about this subject area but a little searching suggests that a s.994 petition is governed by The Companies (Unfair Prejudice Applications) Proceedings Rules 2009 (link here).
In there, you'll find a link to the Schedule which is the form prescribed for a petition to the court. It will need to be accompanied by a witness statement to support the petition.
As for amending a witness statement, that would require an N244 form at a cost of £255 though whether permission is required from the court depends on whether the witness statement has been filed and served on the other side.If you have a question about the voluntary termination process, please read this guide first, as it should have all the answers you need.
