Creating a Deed of Variation

Hello all
I'm about to submit the IHT400 and associated schedules. I need a DoV to adjust the intestacy. I'm trying to create one myself, based on the check list. However i really would like to see an example of how it should be laid out and how changes in distribution are documented. However i cannot find an example anywhere, does anyone know where i could find this or offer any guidance please?
