    Hi I married my wife in 2002, and her children wanted to change their surname to mine, my wife visited the registry office, and they attached a bit of paper to the birth certificate, with their name change, now my son who has been working for a national company for 7 years needs a copy of his birth certificate, or passport (that was in my surname, but is lost), and can't find his birth lines either, and the duplicate still has his original surname on it, were trying to find out what the paper they attached was, and if we can get a copy.

    He's now worried he will lose his job, as he can't prove he's a uk citizen without these

    Did this happen in the UK? What you describe would not be UK practice.
    Here there is the register - which does not get changed, and there are certified copies of the entries in the register. It would not be possible to note any alteration in the register, nor sensible to issue any paper with a note attached.
