Voluntary termination

Help!!

I have contacted my company (toyota services) about VT using a template from here and they confirmed that I can do it.
I got an email today and they want me to sign a form before they go ahead with it. I’ve read on here to ignore their paper work though.

do i sign the paper work or not?
