Help housing claim

I have a somewhat complex situation with my Housing Association and wish to bring a claim,
for Breach of contract
Negligence
duty of care
is there anything else I could add, and does anyone know anything about intentional infliction of emotional distress,
all help in how I can word my claim form would be greatly appreciated, this is more than a housing disrepair
