Does anyone know what should be included in the section worded give the address where the service effected, include fax or dx number, e-mail address or other electronic identification

Given that the documents need to be served on the third party and the defendant the section following then states;

being the

claimant

or

defendants

or solicitors

or litigation friend

but given that the third party is a bank it is not catered for. this form is terribly formatted and messy for what should be a very simple process.
