Fully pleaded defence and counter claim template? Urgent

Hi
A couple of weeks ago I managed to get a CCJ set aside.
The Judge gave us 14 days to file a fully pleaded defence and counter claim. Is there a format for this? Ive no idea what it should look like? Is there a template or headings I should follow?
Please can someone help? Ive looked around the site and cant find anything that fits?

Acknowledge Claim - within 14 days from Service

Defend Claim - within 28 days from Service (IF you acknowledged in time)

If you fail to Acknowledge the claim you may have a default judgment awarded against you, likewise, if you fail to enter your defence within 28 days from Service.
