Who to name as Defendant in judicial review against County Court?

I need to file a judicial review in respect of a Circuit Judge's decision but I don't know what name and address to put on the claim form for the Defendant, so can anyone help with that?

I'm also including a human rights claim in my judicial review, in respect of the Circuit Judge's decision but also a High Court Judge's decision. Do I need to name the High Court as a 2nd Defendant, or is there a way of naming the first Defendant which includes both the County Court and the High Court?

Acknowledge Claim - within 14 days from Service

Defend Claim - within 28 days from Service (IF you acknowledged in time)
