pcp vr after one year into a 4 year contract

hi I have a pcp contract and need to give up my car due to ill health
and not sure how to go about handing the car back as it is sitting at the side of the road and not being used .
I was wondering if anyone had a template letter to send to Barclays bank stating I wanted to return the car any help would be great.
I'm terrified about telling the finance company I'm returning the car
