Help completing directions questionnaire N181.

Hi all. My elderly mother in law has received a directions questionnaire N181 from county court money claims office in relation to her late husbands company which has now dissolved (over 10 years ago). The questionnaire is so confusing to me,I have absolutely no idea how to complete it. The claimant is trying to claim for loss of hearing from working in the company in the 70's.... The claim itself is against another company and actually no mention of my father in laws company in the actual defence. Can anyone offer any advice on how to complete this form? Thank you.
