Hi guys I’m new to this site
I need some help with a deed poll. Specifically the LOC021 form for a statutory declaration.
As I understand, I need a witness to fill in this form in the presence of a solicitor. What I’m unsure about is the section where it asks for document name. There are two fields which need filling in and I don’t quite understand exactly what I or my witness need to write in there. “the certificate of (NAME THE DOCUMENT SHOWN AS PROOF OF CITIZENSHIP) and the second field “now produced and shown to me and marked “B” is the certificate of (NAME THE DOCUMENT SHOWN AS PROOF OF CITIZENSHIP)
