Please advise me on CPR 8 application notice

Hello.
Received a letter from a solicitor stating hes made a CPR 8 request against me. no notice. no court forms. no reference number. no stamp. he asks how I will defend it. How can I if I don't know where it is, what ref number. he also tells me he did it 6 months ago. is there an obligation on his behalf to pass on details or to notify me correctly.

Acknowledge Claim - within 14 days from Service

Defend Claim - within 28 days from Service (IF you acknowledged in time)

If you fail to Acknowledge the claim you may have a default judgment awarded against you, likewise, if you fail to enter your defence within 28 days from Service.
