Just a couple of points
If the user name is in anyway related to your real name I would ask one of the admins if it could be changed as it seems that lawyers from some of these firms trawl the forums and can use what you have posted as evidence against you
If you want to pursue reclaiming bank charges be very careful. I am not saying it can't be done but a poster on here recently tried and has been stung with a bill for £6000 costs. There is however no harm in starting the process and going as far as the FOS if needed - just think twice before launching into legal action.
The problems with O/D's is that many banks leave them running long after they have stopped being used and will claim the cause of action for SB is from when they sent a formal demand for repayment
Can you tell us roughly how much this is , is it hundreds , a few thousand, between 5 and 10 thousand or over 10 thousand
I think you may have some comeback as you were only 17 and they should not have let you go overdrawn ( I think)

Have you sent the acknowledgement of service to the court as well as the claimant ? If not you need to. ( you can send the form from the response pack or do it online on Moneyclaim online ).
Also I don't think that would suffice as a request under CPR 31.14 so I would do a dedicated letter. The POC is very vague and only mentions the contract, so enforcing anything other than the contract request through court would be tricky, but you may as well add the notice of assignment and default notice on there and see what happens. ( letter example CPR 31.14 Request Letter )
Now when you say the debt is disputed, have you a formal complaint in progress with Lloyds/Ombudsman or you just wrote to Arrow/Restons telling them you disputed the debt ?
Do you recall when Lloyds closed the account ? Was it before June 2011?#staysafestayhome

Acknowledge Claim - within 14 days from Service
Defend Claim - within 28 days from Service (IF you acknowledged in time)
