      Try this

With reference to our recent correspondence, I write to advise you, once again, that I am now in a DMP with CCCS and have been for the past 4 months, following advice taken from yourselves..
Please be advised that your continued letters and telephone calls to be harassment and as such will be reporting your company to Trading Standards and the OFT should this harassment continue. OFT guidelines are very clear on the matters of debt collection, and I enclose a copy for your information.

Should you have any queries, please contact CCCS on/at wherever.

Just codged this up, might need a bit of tweaking.

        Re: help please

and you could chuck a bit of this in as well, if you think its relevant

" I DO NOT give you permission to contact me by telephone, and request that my telephone number is removed from your records immediately.

I would also like to make you aware of The Office of Fair Trading Code of Guidance in which it states: putting pressure on debtors or third parties is considered to be oppressive. This includes: Ignoring disputes about whether money is owed and refusing to freeze action if the debt is in dispute. I have included copies of the Guidance for your information, with the relevant items highlighted for ease of reference.

I therefore hope to receive your full co-operation in this matter and would like to request a written response to that effect."
