Mediation Scheme - Reply Form

The above form was included with my AQ (received from the court) - can anyone offer any advice on how to complete it please?

Is this just a standard form or have you been ordered to complete it?

If it's just a general form then you've tried mediating with them yourself alraedy and it hasn't worked.

It's a standard form which was included with the AQ. I haven't tried mediating with them personally apart from my many requests for a settlement.

Unless ordered to by the court there is no requirement to complete and return it. Often they are just stuck in the AQ envelope by the clerks.
