Who was the original creditor?

what sort of debt is this?

what was the nature of the dispute?

I never did get to post up the document but tragedy over as they have now asked if we are willing to agree to no costs as they want to drop the case. They made many mistakes. We will say we agree only if they remove entry from credit file.
Any comments welcome please.
