In your call tomorrow I would ask whether they record calls which they would need to do to in some form to support their assertion that
"Your P45 was issued when requested in the call on 11/4/2018". I
f so ask how is this done i.e. by direct recording in which case if they hold them for nearly 11 months you would like to be given a copy of them in order to prove their view that the calls you made in March did not during the course of them request your P45. Alternatively were they by means of hand-written notes during/after the call? In which case you would like proof of the notes taken, which effectively should be date and time stamped. Effectively your are trying to find out how they can assert their point is correct if they cannot also disprove your argument about the content of the calls in March.

What you need to weigh up is the cost of getting a legal professional to prepare your paperwork, the cost of filing the claim and in the event you lost the court may make an award against you for the other sides losses (but not their legal fees) against the amount you are actually claiming.

I do not provide advice by PM although I may on occasion ask you to send me documents this way but any related advice will be provided back on your thread.

I do my best to provide good practical advice, however I do so without liability.
