Letter before claim from PRA

Hi Everyone, sorry if this has been asked before but I couldn't find a clear answer. I received a Letter before claim from PRA Group and will be sending them a CCA request letter (with the £1 fee). My question is, do I have to or should I fill out any of the information in their reply form which was sent with the letter before claim and send it back with the CCA request? its seems a pretty standard form, tick box A B C, income expenditure, BOX I i need more documents or information etc just not sure if its needed at this stage and to send the CCA request? thanks in advance.
