Sorry for short notice but the letter was received on 16/05/2019. This is my first post on here, I need some help. I have received this letter about attending court for a N61 form. I have no idea what the debt is or when from. I have attached an image of letter but due to size this is all i can fit into image. The letter is from HM Courts & Tribunals Service (Bailiffs office Liverpool county court.)

Acknowledge Claim - within 14 days from Service
Defend Claim - within 28 days from Service (IF you acknowledged in time)
If you fail to Acknowledge the claim you may have a default judgment awarded against you, likewise, if you fail to enter your defence within 28 days from Service.
