    Issue Date: June 2017
    Have you Acknowledged the Claim?: No
    Total Amount Claimed : £500
    Claimant’s Name: Lowell
    Solicitors Firm: Lowell
    Original Creditor: O2
    Original Debt (eg. Credit card/Loan/Overdraft) : £400
    Particulars of Claim: ( Please type out in full excluding names/account numbers/exact amounts ): Account started and then left to lapse due to mental illness, went through multiple rungs and then ended up with a CCJ. Was issued default judgement due to lack of defence.
    Is the debt Statute Barred (have you had any contact with the creditor or claimant over the last 6 years?): No
    List any letters you have sent (eg: CCA/ CPR ): None, however emails have been traded.
    Any Other Information or Background Details: The debt has been written off by Lowell due to mental illness. I have received a message from them saying "should you make an application to the Court to set Judgment aside, our client would not object to the application and will indicate this to the Court.". What is the correct form and process I should go through to apply for this?
