Shouldn't you be getting your solicitor to complete the DQ rather than asking us on here?
It's not really helpful to simply say you need help with the DQ because the questions being asked are not that difficult. Also, if you are expecting us to complete the forms for you then we won't because we don't know your case fully. So you are going to have to specify what exactly you need help with.
Before you answer that question, you need to help yourself first so I suggest you read CPR 28 (click here), the Practice Direction (click here) and HMCTS guidance about cases on the fast track (click here). These references should give you everything you need to complete the relevant forms.

Acknowledge Claim - within 14 days from Service
Defend Claim - within 28 days from Service (IF you acknowledged in time)
If you fail to Acknowledge the claim you may have a default judgment awarded against you, likewise, if you fail to enter your defence within 28 days from Service.
