Got a claim form from CPP I have no idea when or who I took it out with so what do I put in the box as to why I want compensation

I've also got one of those forms a couple of days ago. If you didn't know you had the policy yet you were being charged for it, that should be a good enough argument.

Like many others, I don't remember taking it out, I have a vague recollection of filling in a form listing all my credit cards around 2001, I think it was headed CPP but I was under the impression it was part of having a "Premiere" current account with the A&L. I don't recall signing up for the ID theft policy (according to their forms, I had both CPP and ID theft).
