have you done an I & E ? sheet you should also enclose as much proof as you can on any debts or bills you are having difficulty with and be totally honest with them and enclose a covering letter explaining why you think you qualify as a hardship case.

Firstly, credit card charges are not covererd under the OFT waiver, so you should start your claim for those charges straight away. Natwest credit card were relatively easy to get the charges back from, I did my OH's in a matter of weeks. TonyB - v - Natwest Credit Card ***SETTLED*** - Legal Beagles

If you are in financial hardship, you need to be able to prove this to the bank and you may be able to get the bank charges sorted out more quickly under hardship rules. If you have all the charges to hand, then you should be able to start your hardship claim stright away.

Have a read around the hardship forum, and fill in the Income/Expenditure sheet. You need to have evidnece to back up your hardship claim, such as mortgage/council tax arrears etc. Each hardship claim is differnet so there is no template letter as such, your letter needs to be tailored to fit your individual circumstances.
