No need for any particular template at all, just something like this would do...


Auntie's name deceased.

Statement of assets and liabilities as at date of death (insert date here)

Assets
Nil
(see attached statements as evidence)

Liabilities

MBNA Credit Card £x.xx
Bank Credit Card £x.xx

etc etc and include statements if possible as evidence

Net value of estate = £-x.xx

Executors sign as confirmation and off you go....


Total Assets NIL
Total Liabilities £x.xx

etc etc
