Cap 1 Help Needed

Howdy fellow folks

I urgently need to check out something about their application forms.

What I need to know is if the mail-shot type application forms that periodically get sent out to all and sundry have the executing signature for CapOne already preprinted on them. Does anyone have any recent or older original copies of this type of form, or has anyone got a similar doc. from a magazine/shop leaflet dispenser etc.?

Any help would be greatly appreciated!!

I used to have a Cap1 card so will have a shufty, but tbh I wouldn`t hold your breath. Will let you know if I find one.

I have a copy of one that has the stamp on it. It is from March 04, will post it if it's what you need Enaid x
