Draft Standard Directions - Fast Track

Hi all. Hoping you can help. I am filling in the directions questionnaire and drafting the order for directions for a proposed allocation of fast track - I agree with this but would prefer for it to be in the County Court not High Court. I have a couple of questions.

1. If I note down a witness on the directions questionnaire, do they have to attend court or can they provide a witness statement only? If I don't note down anyone, can I call a witness to trial?
2. For the draft directions, do I have to share them with the other side first for agreement or can I just draft my own. They have not approached me yet and the deadline is very close so I am wary of doing this.
3. For the draft directions, do I leave the dates and name of judge, etc. empty? Or do I propose dates for disclosure of documents, etc.?

Any help is appreciated and sorry if this was already covered in another post.

A witness should attend court and be prepared to give evidence if asked to. There are a few valid reasons why a witness can refuse to attend the trial
A party can change or add witnesses as the case progresses
