Re: Attachment of Earnings
Who do they say the original creditor was?
HMRC and DWP can directly apply for an attachment, other creditors need to apply to the court that made the original judgment. Have you checked your credit file for any CCJs.
Either way, you must fill in the form (although if there is insufficient information to identify why the order is being made, you can ask for that prior to completing the form) , the lack of a pre-paid envelope makes no difference.

An attachment of earnings order can only be applied for after obtaining a CCJ. These 'random' forms are quite common when people get CCJs without being aware of them, because they've moved or didn't receive the court papers. When the creditor sees that no offer of repayment has been put forward, they often decide to apply for an attachment of earnings to make you pay. In some cases it may be able to apply to have your CCJ set aside if you didn't receive the court papers to start with but you'd also need to have a suitable defence.
If you don't return the N56 form within eight days, the court can order you to fill one in and you can even be ordered to attend court for questioning. At the very bottom of the form you'll find item 10 where you write in your repayment offer, you can also tick the box saying you would like a suspended order. The most common reason to request a suspended order is that if your employer receives this request, it could affect your job. In most cases the court will agree to a suspended order provided you make a reasonable offer simply because if you were to lose your job, you would no longer be able to make repayments, nor could the creditor obtain an order if you are unemployed.
Attachment of earnings orders are a common way to get employed judgment debtors to start paying their judgments. You should ask the court that sent you the form to provide you with the judgment details. You can also search the public record for CCJs for a small fee: http://www.trustonline.org.uk/search-yourself
