First rule is do not identify the driver, use "the driver.........." etc* and a name is not helpful

A brief description would help, the crystal ball is a bit hazy.* Post up a* copy of the PCN, redacted of identifying data but leaving dates.* Rather than post here use an image posting site, such as imgur* and link to it from here
