Consent order for CCJ remocal

Hi all
I am planning to apply for consent order to remove a CCJ. I spoke to the court and they said I will not need N244, and that I just need to apply in writing, stating what all parties are asking to make a court order for and get it signed by all parties.
Is there any templates or examples that I can look at? I am not quite sure how to write it
