    Deed Pol form LOC-20 jargon

    Hi I am struggling to understand the English on page 2 of LOC-20 (05.15). I wish to change my surname only

    "I have subscribed my adopted and substituted" I have ticked the check-box for surname and added my intended new surname, so far so good but the second line- "and also my said old" the two check-boxes for forename and surname and the box "of (old name)" are unclear as I am not changing anything else.

    Can anyone advise me what I am supposed to enter into the boxes at the top of the page please?
