Correcting Wrong default Date

Hello! I'm sure I've seen a template letter for this, but i cant seem to find it.
Very short story - earlier this year Joanna Connolly and her crew got a Hoist claim chucked out. Ive just checked my credit file and the Mercers default notice that Hoist relied on as evidence is dated June 2013, but Hoist has registered a default date as Dec 2013.
I want to write to Hoist and get them to correct the record, since it would mean a massive boost to my credit score as i want to remortgage.

any help appreciated! tks. ADD
