Trouble with BT

Hi
I am a member who has not posted befor.
At the moment I am beginning a dispute with BT who are trying to charge me for 6 months of payments after I moved to Plusnet.
I am aware the rules have changed with regards to a SARS but I was hoping to find a template.
In the past I have reclaimed mortgage, bank and credit card charges, I have reclaimed PPI and also I have managed to get a default removed from my daughters credit file placed by EE,
The adventures continue.
