help filling in injunction form N16a

hi ,I'm trying to take out an injunction on my neighbour and I'm having trouble filling in form N16a,can anyone help me. what do i tick in the first 3 boxes, and what do i put for questions 1-8.my neighbour has already been convicted for assaulting me and last month he damaged some of my property and has been arrested for that. we live on the same estate so its very hard not to bump into him. because he's already been convicted for racial abuse and again been arrested for damaging my property i also want a power of arrest on the injunction. thanks for your help.
