Paranoia is often a symptom of psychosis. I was watching a programme on tv the other evening about serial killers and a consultant neurologist from the USA, who has done a long-term study of schizophrenics, often blamed for murders, found, from MRI scans done on their brains, that the brain tissue itself is deteriorating and, over a five-year period, patients were losing up to 25% of their brain tissue. Quite often, it is their cognitive functions that are affected. It's made me look at one of my neighbours, who has schizophrenia, in a completely different light.
