Having put a caveat in place it was followed by a warning and an appearance was made resulting in a sealed caveat. In this sealed caveat were over 40 major concerns made known. What happens next? Who is responsible for the next move? Any direction would be helpful.

If an Appearance is properly entered, then the Caveat remains in place preventing a Grant of Representation being issued, and it would be for the parties to agree how to proceed between themselves.
Agreement between the parties could result in withdrawal of the Caveat and issuing of the Grant, or could require the party seeking a Grant to apply to the Court for removal of the Caveat and issuing of the Grant
. The Caveator can also seek further orders from the Court on how to proceed with the matter given the stalemate in the application

I've just looked back over your previous thread, and I can only recommend you run this past a contentious probate solicitor.
You said you couldn't afford a solicitor, but there are contentious probate solicitors who work on a no win no fee basis.
They generally have a free initial consultation which will give you a good idea of your chances of winning.(google or perhaps https://justbeagle.com/)
Be very careful before entering this minefield unassisted.
