I'm new to all this and I have sent a email to vt my car

Am a bit worried as to if I have done it right so want some advice please

The template from rob wouldnt download for me so I got another from elsewhere

Can you let me no if this is correct

Thanks for the reply appreciate it.... I sent that sunday night what would be the normal time frame before I get a reply? Also my tax and mot run out on the 1st of august would I need to renew them both of not picked up before that date
