sorry for late reply, i was at work. i don't really know how to send you the form!
on the form says "customer inquiries form" and it is asking me to make representation in writing to challenge the penalty fine.

[MENTION=5354]mystery1[/MENTION] hello mystery1 i have received a letter regarding the POPLA and they have rejected my appeal. it says the parking attendant had gone inside the McDonald and ask for the driver before issuing a charge notice. and it stated they have not chosen to participate in Ombudsman alternative dispute resolution service.
