How would it work if Council planning permission was granted for something eg ground floor extension if the freehold terms (owned by different live in owners in proportion to property size) do not allow any such extensions?

Planning permission is solely that.
It does not give the recipient the right to do something otherwise not allowed
An example would be a person obtaining planning permission to build on someone else's land. Although the planning authority have granted permission, it does not mean the recipient can go ahead anyway.
