Change of Name Divirce

I got divorced and changed my name with a solicitor although there does not appear to be any record with any deed poll register. I got married again in my new name. That marriage failed although we have never divorced. Shortly after my second marriage failed my first husband passed away and I reverted back to my original married name. I don't have any documents. This was 15 years ago. I just changed everything back to my previous name. I now want to divorce my second husband which is in my changed name. How do I go about this. Many thanks in advance
