My employer did not send out an SSP1 form. I wrote to them and they said they sent it out by post and it must have gotten lost. They sent me a blank unsigned SSP1 form with no date. So what can I do about this? I didn't even know anything about this until now. I was unaware that I could claim SSP1 when my sick pay finished from my employer. I suffer from sleep apnoea so was just too tired to address this until now. I have read that the government will back pay for three months but not more. Update: they sent me the form via email and it has been partially completed . So can I hold them accountable for the form being sent to me late and incomplete? The form is unsigned and no date so don't think anyone will accept this. There is a box where it instructs the person preparing the form to sign and date here after printing. Seems like the form has to be printed off signed and dated then sent to the employee, I mean is it difficult to print off a form, sign and date it and then scan it and send as copy by email to the employee? I would think that the signed and dated form has to filed by the employer for their records? This was done by the NHS. Any advice appreciated.

You will need to ask your employer to date and sign the form. All they should do is keep a photocopy of it on your personnel file and then send it to you.
