From N181 - Point D4 & J

Hi there,
I was really hoping if someone can help me with a couple of points on N181 form.
Point D4 as I understand it this needs to have a draft disclosure directions but I am not sure how to actually make this and what to say in it, can someone please guide me on what to do in this point with maybe a short example and maybe a template.
Point J - This section is asking for draft of the order for directions again I am not sure what this really means and how to create this and it will great if someone can please help me with this and with maybe a short example and maybe a template.

Acknowledge Claim - within 14 days from Service

Defend Claim - within 28 days from Service (IF you acknowledged in time)
