I have to put in an N244 Disclosure Notice, seeking disclosure of documents that I need to formulate a claim ... the defendant has declined to provide these, despite me issuing a formal pre-action protocol disclosure letter some months back. I need help filling in the Application ... can anyone assist?

If you need information now from the defendant to help your claim, how did you bring the claim in the first place?
What evidence was it based on? surely you must have had al the info to enable you to bring the claim in the first instance..
Can you give us a bit more background so we can see what has occured so far

Rule 31.16 of the civil procedure rulles (CPR) deals with this.
